Photodynamic Therapy (PDT) for Acne Treatment

The PDT laser treatment targets hyperactive oil glands in your skin, which are the root cause of acne. Laser treatments can also effectively treat deep acne scars. Since acne is an ongoing condition, patients can continue using their current acne treatments, prescriptions, and skincare routines while undergoing laser treatments.

PDT for Acne Treatment

This treatment can smooth out old acne scars, giving your skin a softer and smoother appearance. Photodynamic Acne Treatment (PDT) involves applying a photosensitizing agent to your skin. PDT-ALA can significantly improve moderate to severe acne, including cystic acne.

Before Photodynamic Acne Treatment, the best option for resistant and cystic acne was Accutane. However, Accutane has many systemic side effects, including birth defects, liver abnormalities, mood depression, and dryness and night vision changes for virtually all patients. *Systemic treatments are medications taken orally, injected, or infused that affect the entire body. PDT Acne Treatment offers a viable alternative to other acne treatments without negative systemic effects.

The advantages of PDT include rapid treatment of acne lesions, no scarring, no surgical excisions, and no systemic side effects. Following these treatments, the skin appears refreshed and younger. We recommend 2-3 treatments at 2 to 3-week intervals for optimal results.

It's important to understand that, as with any medical treatment, there are alternative treatments and options to PDT. This therapy might not work for everyone. These typically appear on the face, neck, back, chest, and shoulders. Acne can cause emotional distress, and severe cases may result in permanent scars.

What Causes Acne?

Acne starts when skin pores become clogged and can no longer drain sebum (an oil produced by sebaceous glands that protects and moisturizes the skin). The buildup of sebum causes the surrounding hair follicle to swell.

Swollen hair follicles with sebum are called comedones. If the sebum remains beneath the skin, white bumps called whiteheads form. If sebum reaches the skin's surface, darkened bumps called blackheads appear. This black coloration is due to the sebum oxidizing, not dirt. Both whiteheads and blackheads can persist in the skin for a long time.

Propionibacterium acnes (P. acnes) bacteria, which normally live on the skin's surface, can enter clogged pores and infect the sebum. This causes swelling, redness, and pain.

Infected sebaceous glands may burst, releasing sebum and bacteria into the surrounding skin, leading to additional inflammation. Severe cases can result in larger nodules and cysts forming in deeper skin layers.

Types of Acne

Acne severity varies and can be categorized as:

  • Mild Acne: A few scattered comedones with minimal inflammation.
  • Moderate Acne: A denser collection of comedones along with red, inflamed, pus-filled lesions.
  • Severe Acne: Also known as nodular or cystic acne, it involves widespread, deep, painful lesions that are likely to scar if untreated.

Who Gets Acne?

Anyone can develop acne. It is common in teenagers due to hormonal changes during puberty. Adults, particularly women, can also experience acne. People with a family history of acne are more likely to be affected.

Factors That Worsen Acne

Acne lesions can come and go. Factors that can trigger flare-ups include:

  • Hormonal changes in women, such as before menstrual periods, during pregnancy, or when starting/stopping birth control pills.
  • Oil from skin products or workplace grease (e.g., kitchen environments).
  • Pressure from sports helmets, equipment, backpacks, tight collars, or uniforms.
  • Environmental irritants like pollution and high humidity.
  • Squeezing or picking at blemishes.
  • Harsh scrubbing of the skin.

Acne Treatments

Almost all acne cases can be effectively treated. The goal is to heal existing lesions, prevent new ones, and avoid scarring.

Treatments address one or more underlying causes of acne. Topical retinoids (e.g., Differin, Retin A Micro, Tazorac, tretinoin) can unclog sebaceous glands and keep pores open. Antibiotics may fight P. acnes bacteria. Accutane or hormonal agents (e.g., birth control pills) may reduce sebum production. Your doctor will prescribe medications based on factors such as acne severity, previous treatment results, degree of scarring, and gender. Non-prescription medications may suffice for mild acne, but moderate to severe cases often require prescription treatments.

It's crucial to give any treatment enough time to work, typically 6-8 weeks. Consistency in using medication is key to controlling acne.

More and more doctors and patients  are seeking  to improve the appearance of the aging face not stretching the skin tighter, but filling it from the inside by fillers.
Enter the age of the liquid face lift. A Non-Surgical Facelift is the lifting, plumping, filling, smoothing, and/or re-contouring of the face through the use of injectable dermal fillers, such as Restylane, Perlane, Radiesse, Juvederm, Voluma, Bellafill, and Sculptra.                                                           
The use of Botox in addition to dermal fillers can make for better, longer-lasting results. The final result is a facelift which addresses the lower two-thirds of the face. It lifts the cheeks, jowls and neck to smooth wrinkles and provide a more youthful appearance.

The best candidates for a non-surgical facelift include someone with minimal to moderate signs of aging. Younger patients often begin non-surgical facelift procedures as a way to prevent or to delay the  signs of aging as they are beginning to appear.   
It is common for patients who have had Liquid Face Lift procedures to say that they receive compliments on their appearance without others knowing that they have done anything.

In Some instances, the results can be so uplifting that they change an individual’s life. Liquid Face Lift procedures generally take around half an hour to one hour and the patient can return to most normal routines the same day.
The aim, says Dr. Gary is to restore the skin’s firmness and contours by restoring volume, which results in a healthy appearance. My approach is always conservative, gradual and natural. The end result is a revived, fresh and firm appearance, not an inflated or deformed one.

Botox can be especially useful when dermal fillers have been used to fill in expression lines, such as the creases that form when you smile, as repeated overuse of the surrounding muscles will cause the filler to break down more quickly.
Specifically, you should be aware that silicone in liquid or gel form is not approved in the U.S. for injection into the face or body, and its use in this type of procedure has been associated with many unpleasant, disfiguring, and even fatal complications. Do not get a dermal filler procedure in the presence of any active skin sore, herpes outbreak, pimple, rash, cyst or infection near the intended injection site.
Also, bear in mind that getting laser treatments, microdermabrasion, chemical peels or other similar skin treatments at the same time as your dermal filler procedure is not recommended. Finding a well-qualified cosmetic dermatologist who uses only FDA-approved dermal fillers (in other words, no silicone liquid or gel) is a start.

Dating back to ancient Greek sculptors and later Renaissance artists, the concepts of proportional balance have been a focus of interest to those who work with the human form.  Since that time, we continue to recognize that facial attractiveness is determined partially by the dimensional balance of facial components.

Leonardo DaVinci  presented  Vitruvian Man  and Male Head in Profile with Proportions to  outline his observations of this phenomenon.

Among this work and others, the observation is similar:  when the dimensions of one facial element  are mathematically harmonious (based on measured length,width, etc.) with the dimensions of other features, we usually find that face attractive. The profile head contains notes on proportion Leonardo not only produced studies of the proportions of the human head, but also a table in which he recorded all possible types of noses. In addition, he combined various forms of foreheads and chins as well as different types of noses and mouths. 

This ratio is referred to as the Golden Ratio,  the Divine Proportion, or Phi.  It actually measures out to be 1.618 : 1.  Interestingly, this same ratio appeals to the human eye in all forms.  For this reason, corporate logos ( Apple, etc.) are actually designed following this same ratio.
                 

These faces above are absolutely identical with the exception of the dimension and shape of the jaw.

For example, a nose with a specific shape and dimension, when placed on a face with a weak chin can often look too large.  Thereby,  the face  is felt to be unattractive. 

 By contrast, the exact same nose balanced against a strong jaw can look appropriate and prove the entire face to be attractive.


 
Renowned Italian artist and Renaissance master Leonardo Da Vinci was born in Vinci, Italy in 1452. He passed away at age 67 in 1519. His genius among others included painting, engineering, mathematics, philosophy, and inventing.

Another example to illustrate proportional balance and youthfulness can be found in the relative size of the nose and eyes. Relatively large eyes with a relatively small nose is considered “infantile” in nature as this proportional feature is typically found in youth. 
 

Studies have shown that both sexes perceive this proportional feature to be more attractive than a relatively larger nose and smaller eyes.  A similar phenomenon is found in the peri oral region (area around the mouth).  Here, we typically measure the vertical height of the upper lip complex in a young person to be approximately one-half or smaller than that of the lower lip complex. 
 

 This can be appreciated both at rest and with facial animation.  As a youthful person speaks, the upper teeth are visible and are showcased with movement of the lips.  Similarly, with smiling, the entire upper teeth and upper gum line are also typically visible in a younger face. 

 With aging, however, the upper lip lengthens and we find with measurement, that the the vertical height of the upper lip complex becomes comparable in height to the lower lip complex. 

 In this process, the upper lip begins to obscure the upper teeth.  As an older person speaks,  the lower teeth are now visible, rather than the upper teeth.  Likewise, when the aging face smiles, the upper segment of the upper teeth and the gum line are  typically hidden, as well.  This relatively subtle change in proportion of the peri oral components can yield the unconscious perception of an aging face.

This concept of proportional balance of features is very important in approaching the treatment of the face.   By addressing an imbalance and bringing the facial elements more into harmony, Dr. Gary renders the face more attractive and/or more youthful.  This may be accomplished by reducing the size of a structure that is too large or too long,  by enlarging the surrounding elements that may be too small, or sometimes by doing both.
