Traction alopecia is a type of hair loss that results from subjecting the hair to excessive pulling force.
It is usually caused by overstyling of the hair with tight buns and braids, but can also be caused by trichotillomania (a mental illness that causes people to compulsively pull out their hair) and excessive use of hair chemicals. It is usually a short-term condition and can easily be reversed if the action causing it is stopped.
If detected early, traction alopecia is reversible if diagnosed early. However, permanent hair loss can occur too slowly for immediate detection. Hair loss is often occurs in the front, but is also subject to the immediate adjacent area to where the hair is being pulled and damaged.
Traction alopecia does not respond to medical treatment such as minoxidil or finasteride since by it's nature, the hair loss is different then genetic hair loss.
The only way one can treat traction alopecia is with cortisone injections. In the later stages of the disease, traction alopecia is irreversible.
Patients with longstanding disease develop a scarring alopecia with fibrosis of the hair follicles. In those cases, hair transplantation and flaps have been described with good results.
Follicular unit hair grafting has been identified as practical solution for treating longstanding traction alopecia. Hair transplant surgeons are reportedly seeing a rise in the number of patients with traction alopecia and the results have been good to excellent but hair transplanting should not be done until the patient is under control without any further traction alopecia.

It is clear to me that history and examination skills remain at the very core of my dermatology practice. Older Dermatologists like myself rely on history and physical to a greater degree than the younger dermatologists. I think that the pendulum has swung too far towards all these cosmetic procedures where the younger dermatologists are just doing cosmetic procedures and not listening to their patients. This is an important point because if the Dermatologist does not spend enough time with the patient during the consult the cosmetic procedures will probably not work.
